CONFIDENTIALITY AGREEMENT
This Confidentiality Agreement is made and entered into by and between Dentist Dr. Dt. Emin Celalettin ÜN (Address: Atatürk Bulvarı, Hastane Caddesi No:36/A, Kocasinan / KAYSERİ) and ........................................................................ operating in the field of ...................................................................

Dentist Dr. Dt. Emin Celalettin ÜN shall hereinafter be referred to as the SERVICE RECIPIENT, and ……………………………………… shall be referred to as the SERVICE PROVIDER.

  1. The SERVICE PROVIDER shall comply with the provisions of the "Law No. 6698 on the Protection of Personal Data", in particular the "Convention No. 108 on the Protection of Individuals against Automatic Processing of Personal Data", while using the information and personal data transmitted to or acquired by the SERVICE RECIPIENT as a requirement of the service provided to the practice of the SERVICE RECIPIENT, The SERVICE RECIPIENT shall act in accordance with the provisions of the "Convention on the Protection of Human Rights and Dignity of the Human Being with regard to the Application of Biology and Medicine", the rules of professional ethics of medicine and other ethical rules related to the subject and the measures required to be taken by the SERVICE RECIPIENT.
  1. The SERVICE PROVIDER is obliged not to process and share the personal data, especially the personal data of private nature, of the patients and their relatives, which it has learnt during the period of providing services to the SERVICE RECIPIENT, without the consent of the SERVICE RECIPIENT. This obligation continues indefinitely after the termination of the service relationship.
  1. This contract covers the personal data of the patients as well as the special medical techniques applied by the SERVICE RECIPIENT. In this context, the SERVICE PROVIDER accepts, declares and undertakes that it will not share the special medical practices and techniques applied in the practice with anyone and will act in good faith and honesty by complying with the rules regarding unfair competition.
  1. The SERVICE PROVIDER, who acts contrary to the obligations set out in this undertaking during and after the service relationship continues, accepts, declares and undertakes that the legal and criminal liability in this regard belongs to him and that he will unconditionally compensate the material and moral damages suffered by the practice and its patients in this case.
  1. The provisions of the "Convention No. 108 on the Protection of Individuals with regard to Automatic Processing of Personal Data", the provisions of the "Law No. 6698 on the Protection of Personal Data", the provisions of the "Convention on the Protection of Human Rights and Dignity of the Human Being with regard to the Application of Biology and Medicine", the Labour Law and other relevant legislation shall apply in matters not provided for in this agreement.


  1. Ankara courts and execution offices are authorised in case of disputes arising in the implementation of the contract.
